Have you considered adding this survey to your web site? A form could
easily be added to the site as a option for web-using visitors.

When I visit museums, I usually dont leave until I have to. At that
point, I'm often reluctant to stop for a survey. A form on the web would
be much more convenient for people like me. If someone hands me a note on
paper with the URL as I'm leaving the museum, its no bother to visit the
web site later and complete the survey.

Furthermore, if you want to ensure that the person actually visited the
museum (and is not just joking around on the web) the survey page could
be entirely separate from the museum home site. With no links to the
survey, only those who were handed the survey-URL in person would know
about it.
