Hello, List!

This weekend, our small historical society is hosting a symposium for all the county public history organizations.  This is the first time anyone in the county has done something like this, and it is shaping up to be a big event as almost all invitees will be attending.  Of course we would like some mechanism to receive feedback.  Can anyone recommend a form that may suit the purposes for this kind of event?  It doesn't have to be an exact match, just something we could use as  a springboard to create a template of our own.
