I'm not sure if this will be of help, but we have a loan request form online that we ask prospective borrowers to fill out. You can find it here: http://anthropology.si.edu/AnthropologyForms/PreliminaryLoanRequestForm.html .
There is also more information here: http://anthropology.si.edu/cm/borrow.html .

Subject: [MUSEUM-L] Good Loan form
Hello talented and wise list serve members,
I am looking for a good loan form that one might use in requesting original materials loaned from an archives. I am not really satisfied with ours - it seems incomplete in terms of commitment on our part in outlining the agreement beyond logistics. What is "standard"? Does anyone have one they recommend from their own facility or one you recommend that is accessible on-line? Thanks so much!
Best,
Laurie Sedicino
