Connie, this is my personal experience with this issue:

Financial reimbursement for unused benefits is at the discretion of the
employer, however I have rarely seen an organization give the cash equivalent
to someone who refuses insurance.  The only time I have seen this is when I
was contracted mid-fiscal year into a position that may or may-not become
permanent.  They gave me an added flat sum each month and basically said,
"Here ... this extra $$ is to cover the benefits you would have had as a
permanent employee."  When the job became permanent 10 months later, my pay
was adjusted to reflect my new benefits package.

The difference here, I think, is that they knew I needed insurance.  I did
not "opt out," I was just ineligible for the program because I was a contract
employee, not permanent.
