They might be the same, and they might be different -- as always, results and opinions may vary.  *smile*

For me, I've always thought one to be the legal document transferring ownership (deed of gift), and the donation (accession) form being the acceptance of that gift into your collection.  The Deed of Gift can have basic information as required by the IRS for the donor's tax records, (and is shared with the donor) whereas the accession form might be much more in-depth, (and is intended for internal use only).   The accession form is where I would record not just the donor information, but also the object's size, age, history, relevant marks, condition,  etc.  I'd even use the accession form (or an attached document) to record those funny little anecdotes about the object that you might want to record for posterity, and/or include in an exhibit label sometime down the road!
