You may wish to look at How to Form a Nonprofit Corporation by Anthony Mancuso
(Berkeley:  Nolo Press, 1994 -- there may be a more current version
available).  It has clear instructions and an accompanying computer diskette
with the forms.

Nonprofit laws may vary by state.   Check with your local arts council to see
if they have a bank of pro bono attorneys to help with nonprofit
incorporation.   Otherwise,
add an attorney to your board who believes in the cause and is willing to
file these forms.
