A good book regarding museum legal questions is Museum Law:A Guide for Officers, Directors and Counsel by Marilyn E. Phelan; Kalos Kapp Press. there is section that covers both copyright and has an form example for licensing agreements. I purchased my copy through the AAM bookstore.
