If you are publishing for an unknown audience you should use the form
recommended by ISO 690-2 "Information and documentation - Bibliographic
references - Electronic documents or parts thereof" in which e.g. a URL
is written as:
<URL:"http://www.cc.gatech.edu/gvu/user_surveys/survey-10-1996">
and the date should be written in the ISO 8601 format, i.e.: [cited
1996-05-16].
If you are publishing for a closed audience you may use any form this
audience would unambiguously understand.
