Why reinvent the wheel?  There is plenty of information about Deeds of Gift
available from museum professionals.  The best sources of information (for
museum forms in general) are

for legal requirements and just plain sound advice:

Marie C. Malaro "A Legal Primer on Managing Museum Collections" (Smithsonian
Institution Press, 1985)
pp 52-138 cover the accessioning process including acquiring title

for examples galore:

"The Museum Forms Book" edited by Kenneth D. Perry (Texas Association of
