The U.S. Department of Commerce/National Bureau of Standards issues
Federal Information Processing Standards (FIPS), among which is FIPS
Publication 10, which lists codes for "Countries, Dependencies, Areas
of Special Sovereignty, and Their Principal Administrative Divisions."
I have Version 10-3, which was issued in 1984, but there is almost
certainly a more recent hardcopy version.
These codes are also used by the National Imagery and Mapping Agency on
their GEONet Names Server <http://164.214.2.59/gns/html/index.html>, where
you should be able to find the latest versions.
