Carrie, the above link (Library of COngress site) illustrates how Special Collections catalogers code information about various preservation actions for books/manuscripts/photographs/whatever. I realize there's alot of library coding described, but I think if you review the associated pages (for the MARC 583 "Action note" field) you'll find I pretty good layout for organizing various types of actions and associated information regarding by whom authorized, when begun, when completed, action done by whom, when item requires review and/or schedule, and such things.
