Any professional producer, location manager, or production company
should provide you with a release form instead of the other way round.
And it seems very odd that they did not do this either before or during
the day of filming. They need the release and not you - they should be
able to fax you something.

A production company filmed something at our building last year
for a PBS production, and now they're going to release the film
nationally. The production company called us this morning and asked us
to get a release form to them by noon today (it's 10:30 here)! We
don't have a release form; does anyone out there have one we could
adapt for our use?
