Our institution has had similar problems with the Lexicon. Our collection includes chemical instruments, not all of which fit the nomenclature. So we have done exactly what other people have sugested, we have simply added to the Lexicon. The nice thing with PastPerfect is, if you don't want to write in the book, some people take aversion to that, then you can simply print out your new and improved Lexicon for others to use.
We also face the same problem as your Doctors bag, only with chemistry sets. We give the object (in your case the bag with all of its contents) one accession number and then each piece is given a letter. The pieces are stored together and are treated as 1 object.
