The Department of Anthropology also has a standard "packet" we send out to researchers requesting access to our collections in storage.  This consists of a Visitor Request form and also a Visitor Services Policy (which includes access regulations.)

If anyone would like a copy of that please contact me off the list.  At this point I could fax it or mail it to you.  Unfortunately right now I only have the Visitor Services Policy in electronic form.
