Pamela and all.....
It can be difficult to keep track of potential references and find them
later on! One thing that I've done is procure a letter of reference or two
immediately upon leaving a position (or acquire it once they know I'm
leaving). That way I have it and don't have to track someone down in the
future.
Another idea is to check with your university/college to see if they have a
placement or file service office. I am fortunate to have a placement file
at the File Services office of Northwestern University -- where they keep my
letters of reference, an updated copy of my resume and curriculum vita.
(When I ask people to write letters of reference on my behalf, they usually
send them directly to Northwestern for my file there.) When I apply for a
job I send out my CV, resume and a cover letter of interest. Then I email
or call the office at Northwestern University and request "x" number of
references and give them the names of the references that I want sent out
and to whom -- which they in turn do within a day or two. It has worked out
quite well!

Here's another question connected to the reference question- what if you

lost track of a former supervisor and the application asks for references,
