There are a few resources out there but we at the Institute for Learning
Innovation (formerly named SLi) have found the use of such cookbook
responses to be problematic --the most important thing is to first think
long and hard about what you are trying to find out.  Randi Korn's book
can be helpful once you are ready to begin developing a questionnaire:

Dr. Lynn Dierking, here at the Institute for Learning Innovation, is
currently working on a book with Wendy Pollock at ASTC that is an
introduction to front-end evaluation.   The book will discuss that
subtlety quite a bit, and it should be out in a few months.
