If you don't mind reading French, I can share the evaluation we had given to visitors during a print exhibit 2 years ago at the Archive Center of Montreal. We basically started wondering what we wanted to know and evaluate exactly and used this preliminary work as guidelines. We also tested a few different versions before releasing the final version. I would highly recommend Beverly Serrell's book Paying Attention: Visitors and Museum Exhibitions. It's not the most recent resource (1998), but the techniques discussed are still standards for exhibition evaluation. It also includes examples of instruments in the back. I'm running off to a meeting now, but I would be happy to send you both some examples of exhibition evaluations later today. The Visitor Studies Association is also a great resource; their journal is available on Taylor and Francis, and their conferences are attended both by seasoned evaluators and other museum staff who are new to this sort of work.
