In response to the posting from indigonights, my bibliography was not published for a fee but only to cover cost of photocopy and mailing. You do the math: 50 distributed for free at AAM in 1996 and since then more than 100 copies requested at about $4. each for copying/ mailing.

Hope this helps explain the so-called fee.
