We use a form titled "Accession/Loan Term Loan Record" for art acquisitions and long term loans (obviously); for donated library books we just use the receiving report that we use for art and our librarian (a volunteer) logs each donated book in a log book.  It works great for us, being a small museum with a staff of 2 fulltime plus 2 part time in the curatorial dept.

I currently work for and organization with a museum, archives and library; each having their own acquisition forms.  Management is looking into creating an all purpose form for all three sections.  Does anybody work for an organization that currently uses a form like this and are there any drawbacks to the practice?
