At the Illinois State Historical Society, we have our membership database in
Access.  Our membership secretary has the program set up in the form field
to have member name, level of membership and expiration date printed out
onto cards.  The cards are printed on continuous feed, tractor fed paper.
The main part of the sheet is the mailing label for the envelope and the
actual membership card tears off of that.  I'll try and find out if she uses
special paper for this or not.
