Try any industrial supply house.  One common nation wide company is McMaster Carr http://www.mcmaster.com/

Search "flat belt" for the material, and "flat belt lace" for the connectors

They carry flat drive belts in up to 16" wide.

Traditionally the belts were leather, but industrial grade leather is getting harder to find as we now raise cattle primarly for meat.
