Hi, all.

Actually, I have been thinking about this very issue for a while now so
I am particularly interested in reading the responses - more
specifically, I am interested in how to construct an evaluation form
for staff to fill out about interns.  Because our interns rotate
through our different departments, they work with almost all of our
staff, so I need to develop something that is not too time consuming
but still assesses the interns' performance and encourages areas for
them to work on.

I can share, however, the evaluation format I use for interns'
evaluations of their internship experiences:

(Usually interns keep journals throughout the internship period, and we
talk periodically about what questions they are thinking about, both
practically (where facts and information might answer their questions)
and reflectively (where there may be no real answers, just further
thinking to be done) and resources to start to answer these questions,
then I ask them to write essays at the end of the internship period.)

Please write an essay of at least three pages, describing what you did
during your internship and then reflecting on those experiences and
observations to figure out what you have learned.  Consider, too, what
questions you have been asking throughout the internship and how you
been working towards answering them.  Please concentrate primarily on
the questions that you have been asking and what you have learned and
secondarily on what you have actually done.

I find that these essays lead to deeper thinking about what they have
learned through the internships, but I would also like to have staff
evaluations of their skills and the work that they have done - a sort
of performance evaluation.  But then would I discuss it with the
interns, write up a summation of the various evaluations from different
staff members, or what?
