If you went to a docter and had him perform a very private and personal
procedure, would you want the release form with a description of that
procedure and your name given to another party without your knowledge for
undetermined use? The documents were shredded based on legal advice received
by our organization, and I feel we corrected a horrible breach of
docter/patient confidentiality.
