I do not know if anyone answered your question off-line or not.

If you go to the downloads page of PastPerfect:
http://www.museumsoftware.com/downloads.htm
and go to the bottom of the page, there are four Data Entry Forms, one
for each catalog: Archives, Library, Objects and Photos.

I would suggest crossing out what you don't need and making copies for use.

Also check out their Discussion Forum under Resources.  A valuable asset.

And, of course, regularly check out the Software Updates for the latest
free monthly update which PastPerfect supplies.

We replaced a "home-grown" package (which I developed in the early
1990's) with PastPerfect and are very happy with the change.
