Following are two sources you might check for condition reports:
 
1.  Basic Condition Reporting, A Handbook, published by Southeastern
Registrars Association (SERA), in 1988, which I believe is being revised at
present (3rd edition).  It contains glossaries, sample forms, and
bibliographies.  You may try contacting:

2.   The Museum Forms Books, 1990, from the Texas Association of Museums,
Mountain-Plains Museums Association.  This is a heavy-duty book including
sample forms dealing with Collection Management, Development, Education,
Security, and Rights and Reproductions.  It should be available from:
