Hello all...

Does anyone have digital format copies of a general form used by museums 
when items are donated? I have several items that I would like to see used 
in new exhibit space at a musem where I work, but we do not have (that I am 
aware of) any forms at all that are used for items to be 
donated/temporarily/permanently loaned to the new exhibition space.

I'd appreciate links pointing to such forms, or digital forms that I can 
edit to meet my museum's specific information. I can be reached off-list at:
