All,

The Carlyle House is a historic house museum with over 50 volunteer
docents.  Beginning this summer, we are instituting tour evaluations for
all docents. What we are looking for is any information, sample forms,
etc. that other museums have used in the past.  Any help would be
appreciated.  Thank you.
