Hi Monica,
I always prefer to use our own Outgoing Loan form when lending to another 
institution.  The Texas Association of Museums publishes a great book- The 
Museum Forms Book, now in its third edition that contains many example forms 
for all sorts of collections management activities.  Here is the link:
