Kim,

A quick Google search brings up several links to the "American
Historical Trails" program that's evidently connected with the Boy
Scouts.  There is a website, www.thecarolinatrader.com, that has
descriptions of the trails and pictures of the medals.  This may be
what you have in your collection.
