If you are on an IBM-compatible computer, I recommend Survey Pro.  You can
find their web page by doing a search - just be careful not to make it one
word, SurveyPro, because that is a different program.
I am still hoping to find something equivalent for use on a Mac.  If anyone
knows of any...
