Hello,

I need to have a generic photo release form to take pictures in a
museum.  Does anyone have a form they can send me? You can either paste
it into an email and reply to the list, (because I don't think
attachments go through) or email it to me at [log in to unmask] or
fax it to me at 630.428.2649.

I am a children's museum exhibit developer that actually works in many
different museums and I need to take some pictures of children in a
children's museum for a graduate school project that I am working on
for Erikson Institute in Chicago.

Your help is greatly appreciated!
