First an apology, sorry I can't provide a full reference but the computer is
"here" and the files are "there"

There is an entire organization devoted to Oral History (you just knew there
must be, didn't you) I expect a simple search of key web sites will provide
a link, as I am unwilling to do it for y'all.

They also have sample release forms you might use a models.
