University Products.  Their latest catalogue (1998/99) lists Accession
(#061-1000) and Deaccession (#061-2000) Registers inside the back cover.  I
guess that they just started carrying them.

Listing:  Acid free, hard cover, perfect bound, 80 2-page spreads,
7-columns, looks like 19 entries per spread.

Each book is 69.00, plus tax and shipping.
