You may have already done this.  However, if you haven't, I would advise
contacting the Society for the Conservation of Natural History Collections
(SPNHC) to see if they have been doing with documentation.  The American
Institute for Conservation (AIC) and, I believe, the AAM Registrar's
Committee have also been working on condition reporting and documentation
projects.
