Check out the protographic fee and usage guides on The New York Public
Library web site (www.nypl.org).  It gives very specific information by
format.

Coming back somehow to the photo/image discussion..

We are currently revising our
regulations and fees for image reproduction. One aim is to adjust
our charter to the current levels, the other is to include reproduction
in different media (e.g. film, CD-ROM, WebPages).
It would be very helpful to get information on how others are handling
the issue, what policies you take particularly in regard to WebPages,
and last but not least what you usually charge for reproduction in
the respective media.
I have seen some very detailed lists for fees, which are very helpful
to get an idea of how to differentiate between the different media an
to see what other ones charge. If you have such list for your own
institution I would be greatful to have a look.
