John, 
You should be able to use your regular outgoing loan form.  You will not
want to make the loan "permanent" (there is no such thing by the way) but
rather make it a renewable loan that renews in 5, 10, 15, or 20 years or
whatever term length makes sense.  You will want to review your state's
abandoned property laws (if appropriate) to conform to them and you will
want a method to periodically check up on the object so having a renewable
loan will allow for that.  You don't want to let either your institution or
the city forget about this object.  Also, you will want to provide for in
your loan conditions how the object will be cared for and what can and
cannot be done to it (ie painting, cleaning, repairs, etc)  Just because it
is large and going outside, you do not just want to loan it and "forget"
about it.
