Hi all,

I've looked in the archives and could not find anything.  Does anyone have
a media release form they use with their volunteers?  Something that
covers photos, audio, and video and preferably is worded to cover a
lengthy amount of time.

Currently our volunteers are signing forms on a case-by-case occurrence
and some of them are getting annoyed that they have repeatedly sign forms.
We're hoping we can go to a form that will cover at least an entire year
at a time.
