When in the rare event someone donates an art object to the Art Center, I
give them, in addition to the "Deed of Gift" form, a "Donor Information
Form" where I ask them to describe how they came to have the object, what it
means/meant to them and their personal collection, and if they can relate
any stories, anecdotes, etc. relating to the object from when it was in
their possession or before they owned it.

The information comes in handy when planning docent tours as well as for
general documentation, provenance research, etc.
