We are working on our administrative procedures for full-time staff. Would
anyone be willing to share the form they use for performance appraisals? If
no standard template is used, what is the basic procedure you use?

Also, do people tend to submit their past performance evaluations when
applying for jobs at other institutions? (I know that Federal employees do
this.)Or are letters of recommendation sufficient?

Any advice at all about performance appraisals in the context of the museum
profession would be appreciated.
