The Registrars Committee of the AAM sponsors a Forms
Clearinghouse, which includes both standard and unusual
examples.  Those who are interested in copies of
particular types of forms should contact Martha Mayberry
at the Mint Museum (Charlotte NC): 2730 Randolph Road,
Charlotte NC 28207; 704 337-2005 (ph); 704 337-2101 (fax).

Martha would also like to receive examples of forms currently
in use.  Please send her whatever you have and she will be
able to disseminate them to your colleagues-in-need.
