What about the short form with an offer to send the
longer if the hirer wishes?  Personally, I can offer a
portfolio, writing samples, clearances, and
transcripts if needed, as well as a CV, but have been
advised to keep the resume down to one tailored page
plus a cover letter.  This is an interesting challenge
with 13 years of work!
