I know requests like this have been posted before, so please excuse the
repeat!  I am looking for models of performance evaluations/annual staff
reviews that you are using at your institutions.  I am new to my position
here, and have found that there is no real history of conducing such
reviews, but everyone agrees that it is time to do so.  You may reply to me
on or off-list, by fax, mail (all shown below), whatever is most convenient.
