There is an example of a "promised" gift form in Marie Malaro's book, "A Legal Primer on Managing Collections", p. 367 (2nd edition).  There is also an entire chapter (XI) on the subject to guide you.

Hello all!  I have been approached by a potential donor who would like to
leave some paintings in her will to the museum collection that i oversee.
THat is just great as we do want these items.  But she is aksing me to
provide the wording that she can use in the will.  Does anyone have a
template for such wording that has a track record of fitting nicely into
the needs of museum collection folks?  I am looking for something along the
lines of a tried and true loan form and other such collection management
forms that we regularly share in this business.  Thanks kindly for any
forms or boilerplate wording you can provide (or warnings that experience
inspires you to offer!)
