The Canadian Heritage Information Network (CHIN) has published a 28pp "Checklist for Licensing Museum Images" (1997).

They have also published other works that may be of interest, including Protecting Your Interests: A Legal Guide to Negotiating Web Site Development and Virtual Exhibition Agreements (1999,43 pp.),  and The Virtual Display Case: Making Museum Image Assets Safely Visible by Peter
Roosen-Runge (1998, 61 pp. ).

Individual publications can be purchased through their web site or entire collection of reference resources by subscription, and can be accessed at http://www.chin.gc.ca/Resources/Research_Ref/Reference_Info/e_reference.html

Right now, CHIN is also offering a 30-day free trial Individual Subscription to these resources, which allows you to access these publications at no charge.  Their home page is http://www.chin.gc.ca/.
