Many academic institutions have the interns write a summary or final paper
in order for them to receive credit...you might ask the intern simply to
forward a copy of this to you instead of having them write something
special, and then use it as an attachment to your evaluation.  Otherwise, a
simple evaluation such as might be used for hourly employees might suffice.
After all, aren't you evaluating them with similar criteria?  Each
supervisor might complete the form (a simple numerical scale with space for
comments if necessary) and you could average out the results.
