The Registrars Committee of the AAM has a Forms Clearinghouse for
its members.  Additionally a Professional Practices Subcommittee is
working on developing a standardized loan form.  A previous Subcommittee
developed a Statement of Practice ofr Borrowing and Lending, which
was published in Registrar Vol 8 No 2 (Fall 1991).  For more information
