Tina,
We do require name badges with first and last names. However we allow the use of pseudenames (usually a change of last name so they can't be found in the telephone directory) where staff are worried about safety. The name must be agreed with, and recorded by, their manager so that if we get a complaint (or complement) about the staff member we know who is being talked about.
