I am not a member of this list, and I hope this message gets through
     to the museum professionals I am trying to reach.  I am trying to
     construct a form for a traveling exhibit for the borrower to sign off
     on (we are the lender).  I am in need of a few prototype traveling
     exhibit forms that I can use for ideas and correct phrasing a.s.a.p.,
     and  would be very appreciative if anyone with such forms in place
     would fax me a copy.

     I would like to take more time on this but we are unexpectedly ready
     to ship next week, and I need to get on this right away.
