Hello listers,

my museum is planning for a temporary exhibition of paintings and other art pieces - these will all be incoming loans, and most of them will be shipped from out of state.  My normal incoming loan form really does not fit this well, as it is intended for use when a patron brings an item to our door.  Does anybody have a from that they are willing to share? One of my director's concerns is in the packing process - we certainly want the art owners to properly pack their items so that they travel safely.  

Suggestions much appreciated!
