For our institution to deaccession anything we have to first document it
(usually listing it with a brief description, with it's current value,
where it came from and how it came to us, and the reasons for
deaccessioning it), then it's deaccession has to be approved by our
collections committee and then board of trustees. After approval it is
noted in the accession files for that artifact that it has been
deaccessioned and if it has been thrown out, transferred, etc.

Always document - you never know when the auditors are going to want to
know what you did with that one item!
